Everything involving any lines should be automated. VAR should be a constant support to the ref on the pitch, and should recommend and correct bad or unsighted calls as needed, in the background, without us knowing. Nothing off limits. The ref should always be sent to the monitor when overturning pens, goals, or for red card situations. If there are any challenges, they should be harshly punished if they are spurious, but in general I feel they would be an unnecessary complication.
Most importantly, PGMOL needs to be reformed or even dissolved, and new standards put in place for recruitment, training, evaluation, promotion, and dismissal of match officials in specific roles, whether on or off the pitch. There should be so many of them that they'd be impossible to know by name. Club surveys should also inform reappointment, weighted against peer evaluation and metrics.
I would also add a second ref. One for each half of the pitch. They can confer as needed. Take away the spotlight once and for all.
